Join Linda Rockwell to discuss vitally important documents related to preparing for significant changes in health. This program is a follow-up to the excellent Sept. 20 program by Dr. Martha Twaddle concerning hospice and palliative care.
Linda worked with JourneyCare (formerly Midwest Palliative & Hospice CareCenter) for 18 years, overseeing services to veterans in hospice care in a 10-county area in northeastern Illinois. She served on Illinois’ POLST Statewide Education Task Force and is a certified Advance Care Planning Facilitator through the Respecting Choices program and has led hundreds of presentations on advance healthcare directives.
